I have no idea what is truly going on or if you are in fact part of the problem and not the teacher. I will assume you are not part of the problem and place blame solely on your teacher and give you my approach.

If this is the case in reality, then take it to your parents and explain each incident. Next would be for your parents, not you, but preferably both of your parents to go to the school counselor and request that you be switched to another teacher. They would have to explain to the counselor and detail exactly why you want out.

I did just this when I struggled with my 11th grade history teacher and could not seem to raise my grade which was not reflective with my of my other grades in all my education. I did in fact get moved out of that teacher's class.

The school year is coming to an end so I suggest you just stick it out for another month and a half. If this was early year or mid-year then I would say make the move.

When i was in school she was pure evil..... you would har her heels walking down the hall, she would come in the door and yell 'kids!' We never knew why because most of us were 18-19yrs old

She would scream at people for random shit every day and then turn around and say sweetly what a nice purse or shoes someone has...then scream again... Its a wonder her husband hasn't killed himself.......we called her bipolar because that woman is not sane enough to teach

She's just trying to help you improve. If she didn't care, then she would let you just fail the class. Then you'll have to repeat it again until you pass. Don't take it personally, it's her job.
