Grade 6 is age 11
7, " " 12
8, " " 13
9, " " 14
10, " " 15
11, " " 16
12, " " 17

Some may turn a year older than each I listed, but for only half the school year, January through June. There are exceptions but they are rare (i.e. parents holding kids back a year very early on whose birth date falls toward the end of a year). The above pertains for the most part.
