Honestly, if it's a medical problem, it's something i suffer from too. My ex and I had sex 16 times before i came finally, and each of those occassion's were extended length's of time, and i enjoyed them greatly.
I never have a problem from it, never even suffered blue-balls, so i wouldnt worry.

If someone is tense and not relaxed, they likely will not orgasm. The mind has a lot to do with ejaculating and whether it is possible. Also, if he ejaculated soon before you had sex, this too would explain why he can't do so while having sex. It is very common for the male body to need some down time before reseting itself. He probably is masturbating a lot and shooting fluids daily which would explain why he can't do so while with you. You can always ask him when was the last time he masturbated. Tell him to stop masturbating for a couple days and I'd bet the house he will when with you.
