Do it as hard or as soft as makes it feel good, its all down to personal preference. Obviously if it starts to hurt you may want to ease up a bit though :D

As for where, again its whatever you like, experiment, there's no right or wrong answers as long as it feels good.

Most people masturbate until they cum, that is the whole point really, although there are people who stop just before. They do this several times a day and then when they finally do it to completion they claim it feels more intense (I think its called being 'on the edge') but again, its whatever floats your boat.
