Answers to numbered questions: 1. Never should be necessary if the boy is healthy and in control of his body. Diapers are used for a very young age and he can be taught to do so himself, first time. 2,3,4: A diabled person who is incapable of doing it himself, is within reason to request assistance but from a caretaker. 5: If arranged in advance and agreed upon by the parties involved in the care of the disabled, then I suppose there is no wrongdoing.
