I wouldn't abandon him. Very few people can handle a friend like that, and if you can don't give up on him. Maybe take a break from him from time to time, but I'm guessing he doesn't have many people around to keep him grounded in what bit of sanity he may have left. Also, you may need to baker act him one day if he gets too bad.
