Everyone should be treated with respect unless proven otherwise. It is always appropriate to use sir and ma'am.

If someone looks 35 or older, I'd call her ma'am, as miss might be considered disrespectful. Calling someone younger than 35 ma'am would be insulting to them as well. Ma'am should be reserved for middle aged and older women. If the person has kids, they might expect ma'am as well, even if they're younger than 35. If a woman in her late 30's is single, she might appreciate being called miss. It's best to avoid both terms when possible to avoid offending someone. I have to admit, I don't like being called sir too much, and that's not even age specific. I find that most people still call me "young man", but the occasional person will refer to me as "that man" or "sir". I still find this kind of jarring. Most of the people who call me "sir" are either women/mothers or kids/pre-adolescents. I don't think I've yet been called sir by a man older than I am! I guess it makes them feel like more of a man to think of me as "just a boy". They might be a bit insecure about their age.

I never call anyone "ma'am," "madam," or "sir." In fact, I hate those words. They aren't really a show of respect any more as just something sales people call you when they're trying to close the deal.
