    Cock and ball torture is a common fantasy. Some people play out the fantasies but on a smaller scale.
    Usually what goes on in our fantasies would not work well (or at all) in real life so some people find ways to "water them down" to enjoy somewhat in real life.

    On a side note - One has to wonder if that is how marriage got started. People thought they were in love and wanted to spend their whole life together. Of course the law made it punishable to split. It sounded good in fantasy i am sure but as we know, reality never has anything good to say.
