It could be a medical problem or a poor hygiene problem.
Maybe this person is just very gassy and needs to have an old school enema, with a bag of warm soapy water. There may be something rotting away in their bowels and it needs to be expelled.

There are conditions that cause fecal odor to be released through the skin. It could be medical.
