Is it normal for a university to lose your application? form?

This happened to me last year. I know I got the address and the department right because I remember double and even triple checking it. I was just wondering how normal this is.

    I bet it happens a lot since universities get a lot of forms.

    We have pretty great admissions staff here and it happens way less often than I'd imagine it would.
