Really want to be castrated, seems like no Dr will do it!

You jumped from cock and ball torture to having your balls cut off which is quite a difference. Some guys enjoy cbt for excitement, but then cutting off your balls means you want to be a eunuch, sexually impotent. Which is it?
Cbt is not castration. If you want to loose your balls, maybe you should be a transsexual.
not worried about pregnancy, testosterone already low, have no need for them all they are is a bother! they get in the road when I sit, and I hate them. just want them gone!
There once existed a man who was active in writing fanfiction for Arthur (the aardvark, in case there's any confusion). I can't remember what his alias was, nor do I know if he's still writing (I'm not brave enough to check), but all of his stories featured castration and were titled things like "Arthur and them get castrated" or "Arthur and them in the gay bathhouse". I don't know if these stories yet still exist, but if they do... find them. They hold the answers you seek
Do you mean you want a vasectomy? They don't actually chop off your balls for that. I can't think of any reason why your doctor would agree to chop off healthy balls.
Why?
If its to prevent pregnancy. Just start taking even a low dose of Testosterone. That will shut down your normal sperm production and you will be effectively sterile (and still be able to ejaculate).
Completely reversible in the future if you take other drugs to maintain your testicle size and future production capabilities (alternately, given enough years on testosterone without those other drugs your testicles will shrink and not be able to recover).
Note also that you need to monitor your hematocrit (red blood cell) levels too, and you might need to start donating blood or do phlebotomies to control that. Too high hematocrit level are associated with thick blood and heart attacks.
