That’s because those things are causing the reaction, but it’s only a way to elicit that reaction to them. It’s well documented in the field of abnormal psych.
“sadist” just like “narcissist” gets thrown around on people way too liberally, majority of people who get called that by others don’t come close to the diagnosis in reality.
