Well, if it’s the sandwich itself, it’s a burger.
If I want to specify the burger has cheese, it’s a cheeseburger
If I want to specify the burger doesn’t have cheese, it’s a hamburger
If I want to refer to the meat portion of the burger, it’s a patty
If I want to refer to a cooked patty intended to be eaten outside a sandwich, it’s Salisbury steak
If I want to refer to the raw meat used to make a patty, it’s hamburg
I’d only call a burger a sandwich if I’m trying to stress that I’m not talking specifically about a patty or the raw meat or a Salisbury steak, but a full burger
“steamed hams” is not in my vocabulary
I usually only ever eat burgers from McDonald's so I guess I call burgers by the name they're assigned at Meccas. Like McChicken or Quater Pounder (;))
i have been known to refer to some as heart-attack-in-a-bun. they're normally the ones that would feed an african child for a month, or one obese guy for 5 minutes
Well, you gave me a good chuckle trying to look this one up:

Burgers and maybe hamburgers if I'm talking to my friends from other parts of the country since they refer burgers as hamburgers.
